Description

A. This position reports to Raleigh Radiology’s free-standing outpatient imaging centers located throughout Wake and surrounding counties.

B. The Patient Nurse Navigator is a Registered Nurse who serves as a liaison between patients and clinical staff. The Patient Nurse Navigator’s primary role is to facilitate and coordinate patient care throughout the imaging process, ensuring smooth and efficient transitions between different stages of radiology procedures and treatment. 

C. The Patient Nurse Navigator is responsible for tracking mammography follow-ups including diagnostic mammography, biopsies, and pathology results, as well as processing patient results and reminder letters for all Raleigh Radiology Imaging Centers.

D. The Patient Nurse Navigator is responsible for tracking lung screening follow-ups including additional imaging, biopsies, and processing reminder and result letters for all Raleigh Radiology Imaging Centers.

E. This position reports to the Senior Manager of Practice Operations with medical oversight from radiologists.


Overview:

1. Implements, maintains and follows policies and procedures of Raleigh Radiology.

2. Performs job responsibilities in a safe, efficient, and professional manner.

3. Assists with or institutes emergency measures for sudden adverse occurrences such as contrast reactions when necessary.

4. Coordination of care: Collaborate with radiologists, referring physicians, patients, referral coordinators, and coworkers to coordinate appointments and treatment to ensure timely and accurate delivery of care.

5. Navigation of Services: Assist patients in navigating imaging as part of their healthcare team, including directing them to appropriate imaging services and explaining the purpose of each test. This can include scheduling follow-up imaging and biopsies for breast patients.

6. Patient Education: Provide patients with clear and understandable information about tests, procedures, and potential side effects. Answer any questions and address their concerns. 

7. Support and Guidance: Offer support and guidance to patients and their families throughout the process, ensuring they feel informed and empowered.

8. Advocacy: Advocate for patients’ needs and preferences, ensuring that their voices are heard during decision-making processes. 

9. Documentation: Maintain accurate and up-to-date patient records and ensure that all necessary documentation is completed.

10. Quality Assurance: Participates in quality improvement initiatives while promoting patient safety and optimal outcomes.

11. Collaboration: Collaborate with other members of the healthcare team to improve patient care and streamline processes. 

12. Establishes and maintains open communication.

13. Maintains a clean, safe, and well-maintained environment for patients and staff.  

14. On a regular, sustained basis, cooperates with other staff members both inside and outside the department in accomplishment of own job duties as well as assisting others in accomplishing theirs; serves as a team player and role model for other employees in the organization, always exhibiting traits of courtesy, caring, helpfulness, and respect; conducts oneself in a service-oriented manner that is attentive, pleasant, cooperative, sensitive, respectful and king when dealing with team members, visitors, public and all employees.

15. Performs other duties and accepts responsibility, as assigned. 


Specific Duties:

1. Contact patients and referrers to schedule breast biopsies and obtain all orders and documentation needed prior to performing the procedure. 

2. Conducts outreach to referrers, providers, and other medical professionals to facilitate patient access to care and services.

3. Track data and outreach to patients to facilitate follow-up for breast and lung recommendations.

4. Performs all tasks required to digitally track, print, and mail results and reminder letters to patients for mammography within MQSA guidelines.

5. Updates reports with pathology results following breast biopsies and lung study patients.

6. Executes MRI prostate segmentation workflow.

7. Assists QC Specialist with any additional duties related to ACR, MQSA, or patient safety tasks as assigned.

8. Follows established protocol for providing service in a professional, efficient manner.

9. Answers questions from patients in scope of knowledge. Directs questions to appropriate personnel if unable to answer questions.

10. Provide annual flu shots to Raleigh Radiology staff


WORKING CONDITIONS / PHYSICAL REQUIREMENTS

Must be able to read without strain printed material the size of typewriter characters, glasses permitted, and the ability to hear the conversational voice, with or without a hearing aid, or the ability to compensate satisfactorily. Frequent, prolonged periods of sitting. 100% of the work is done by computer. Ability to type at 60+ wpm. Wear a headset to connect to the phone. Subject to rotating holidays or shifts.

Requirements

A. Education: Registered Nurse, BLS/CPR Certified (Cardiopulmonary Resuscitation) 

B. Experience: Must have experience in a medical setting. Preference is given to applicants with previous experience in Radiology practice.

C. Licensure: CPR and current RN license 

D. Ability to work independently, make sound decisions based on patient condition, and ability to follow directions. 

E. Flexibility, dependability, and professionalism are required.
