Pharmacists play a vital role in ongoing medication reassessment and, when appropriate, in recommending deprescribing strategies. By aligning medication use with patients’ goals of care, evaluating appropriateness, and identifying opportunities to reduce or discontinue medications, pharmacists are uniquely positioned to minimize medication-related risks and enhance patients’ quality of life. This presentation will feature a case-based discussion, following a complex patient over time to illustrate these principles in practice.
