Administrative Change Form goes electronic

Do you need to make a contact change, a monitoring location change, or have new industrial activities?  Fill out and send in the Administrative Change Form to notify the Industrial Stormwater Program of these changes electronically!  Review your “Facility Details/Virtual Permit Information” page of the Industrial Stormwater Permit Information Access page to make sure your Permit application information is up to date.

Because the Industrial Stormwater Program primarily relies on email to communicate with permittees, it’s important to make sure your facility’s site contacts and email addresses are up-to-date.  Critical messages (annual report mailings, “time to re-apply for permit coverage” messages) will still be sent via U.S. mail in addition to email.

For those who prefer to not use the Microsoft Word version, download the PDF Version instead.

Please note that due to increased responsibility and tasks, data entry staff have a ~2 week backlog of Stormwater Monitoring Report (SWMR) forms and Administrative Change Forms.
