Our Barnum School staff are working hard to make our return to school safe and welcoming for all. Safety is our number one concern as we bring all our students back to our classrooms for enriched, in-person learning. The district planning committee is requesting Barnum families complete this school planning questionnaire to assist the district in preparing for the upcoming school year. Per MDE/MDH guidance as of July 30, 2020, we are currently planning for in-person learning. This scenario may change based on county COVID case numbers.

We are conducting this survey in order to assist in our planning for the 2020/2021 school year.  Because we are requesting that you provide us with private data, we are required under Minnesota law to provide you with the following notice.

· We are conducting this survey to obtain information from you to help us plan for the upcoming school year.  The information in your responses will be very important and helpful to the District as we develop our plans.  We will use the information you provide to us for planning in the following areas: transportation, student learning plans, food service, and cleaning.  We will make public our plans, which may include a summary of the aggregated data we collect subject to applicable law, but individual responses will not be released absent written consent.

· You are not legally required to supply information or respond to this survey.

· There are no legal consequences to you whether you supply information or choose not to supply information.

· School District personnel that may have access to the data for the purposes stated herein may include the School Board, Superintendent, Principals, Human Resources, Business Manager, Administrators, support staff, teachers, athletic director, coaches, coaching staff, community education staff, and ECFE staff.

· We can use and release the data you submit only as stated in this notice unless you give us written permission to use or release the data for other purposes or as allowed or required by state or federal law or court order.
