ANCHOR Care Manager

Hybrid Schedule – Hiring Bonus!

Seeking a full time (37.5 hours/week) ANCHOR Care Manager.  The Advocacy and Navigating Care in the Home with Ongoing Risks (ANCHOR) Care Manager works within the Home Care Department.

Dynamic position assisting eligible elders with recurrent, fluctuating behavioral health needs, to obtain and retain services which will enhance their quality of life and allow them to live as independently as possible with dignity and respect. ANCHOR CM will have a reduced combined caseload of Home Care and ANCHOR consumers to allow for more frequent contact between the ANCHOR consumer and the CM.

Care Manager will co-manage some ANCHOR cases with Protective Services/Crisis with the goal of working with the elder to accept/retain Home Care services to alleviate risk. Position will be supervised by a Home Care Supervisor and consultations with an in-house Behavioral Health Consultant will be provided to further support the Care Manager while promoting acceptance of services with consideration of the consumer’s behavioral health needs.

Behavioral Health background preferred.

Clinical Care Manager – Long Term Services and Supports

Seeking a full time (37.5 hours/week) Clinical Care Manager – Long Term Services and Supports for Elder Services of Worcester Area, Inc.’s contract with Central Community Health Partnership (CCHP) agency. The Clinical Care Manager will provide and oversee integrated care coordination and care management to individuals enrolled in the CCHP’s Long Term Services and Supports (LTSS) Community Partners Program. The Clinical Care Manager will supervise a team of Care Coordinators who provide integrated care management in collaboration with individuals served, community partners, medical and behavioral health providers. The Clinical Care Manager will also participate in trainings, meetings, statistics, program procedure development, and other tasks as listed in the job description.

LTSS Clinical Care Manager shall have at minimum: At least two years of relevant professional experience and hold the following qualifications: A licensed registered nurse (RN),  A licensed independent clinical social worker (LICSW), A licensed mental health counselor (LMHC), A licensed marriage and family therapist (LMFT), A licensed alcohol and drug counselor I (LADC I), A licensed psychologist; A licensed certified social worker (LCSW); A licensed rehabilitation counselor; or A registered occupational therapist. Certified Care Management License preferred, but not required. Excellent communication, documentation, time management and organizational skills required. Driver’s license and reliable transportation necessary.

Community Transition Liaison

Hybrid Schedule – Hiring Bonus!

Seeking a full time (37.5 hours/week) Community Transition Liaison.  The Community Transition Liaison Program (CTLP) is an EOEA state funded program that serves as an expansion to the previous Comprehensive Screening and Service Model (CSSM) Program. The goal of the program is to aid in safe and appropriate resident discharges from a skilled nursing facility (SNF) to the community setting, while working with residents, families, health care proxies (HCP), and skilled nursing facility (SNF) staff.   The Liaison will act as the onsite point of contact for residents, families, health care proxies (HCP), and skilled nursing facility (SNF) staff related to transitions from SNF to the community and to aid in safe and appropriate resident discharge from SNF to community setting.

Duties include but are not limited to visiting with residents/family to increase awareness of services and introduce transition to the community as a potential option, completion of person-centered planning, conducting needs assessment, developing plan of care and guiding the CTLP Case Assistant in implementing plan of care by submitting referrals and completing appropriate applications (housing, public benefits), participating in interdisciplinary discharge meetings, and relationship building.

QUALIFICATIONS:

  1. Bachelor’s degree preferred.

Meals on Wheels Coordinator – West Boylston, MA Senior Center (15 hours/week)

Elder Services of Worcester Area, Inc. Nutrition Program has a position open for a Meals on
Wheels Coordinator 15 hours/week, 9:00 am – 12:00 pm. The primary function of the job is
to provide oversight to the Meal packaging and delivery process for the Town of West
Boylston. The Meals on Wheels Coordinator role is multi-purpose as it involves the
scheduling of the Volunteers (both kitchen staff and MOW driver staff), assist with the
packaging of the hot meals and cold packs for delivery. Print daily route sheets and remove
any late stops called in that day. The MOW Coordinator may be required to deliver if a
Volunteer calls out for the day. West Boylston currently has two Meals on Wheels routes
comprising of approximately 22 consumers each.

Qualifications: Ability to recruit and supervise volunteers required. Must be ServSafe
certified upon hire and annually. Basic computer skills required. (i.e. Receiving/sending
email). Enjoy working with the Senior population. Use of a reliable vehicle including active
vehicle insurance with a current license to drive required. Must be at least age 21 and have
had a Driver’s license for minimum of three years.
Essential Abilities: Must have moderate physical ability. Ability to lift and carry objects
weighing up to 30 lbs. Ability to keep accurate records and statistics, document clearly, and
to communicate effectively verbally and in writing. Ability to maneuver in various residential
settings, including significant walking and stair climbing, while carrying bag(s) containing
meals. Ability to effectively communicate with supervisor and nutrition staff regarding any
issues or concerns with consumers.
