All of the screws and bolts were included with my package. Assembly was straight forward (5-10 min, tops). You don't need to nail anything into the wall. It literally takes just a couple seconds to place in the frame of the door, I've found myself hanging it up several times in the middle of a workout without problem (function of others using the bathroom, not it falling down!).
Design:
It's been perfect for the p90x workouts. I like that it allows for the wide and extra wide hand positions. The hand grips are comfortable and don't seem like they will deteriorate with use.
For the ~150 lb user:
When I purchased this bar, I was concerned by other users' reviews that I would have to wrap socks around certain parts so it didn't dent the door frame. I have had no need to do this, as the bars rest on the foam just fine. I'm ~140lb. It is also very sturdy; I have never noticed it shaking side to side. I would really recommend this bar.
For the 200+ user:
When my boyfriend (215lbs) used the bar (1 time), it did appear to dig in the door frame more, right where the foam pads are in the front. It left a black discoloration on the white paint of the door frame at both of those points and a very slight impression which is only noticeable by running your finger along the frame at that point. He was not confident that the bar (or our door bathroom frame/wall) could support his weight. I don't think the bar would break, but it would probably dig into the frame/wall over time. If you are concerned about the cosmetics of your door, check out the pics other Amazon users posted showing where to wrap socks so it is less likely to cause indentations. I can't really recommend for or against any bar for users 200+ lbs, but know that if you do get this one (most likely any bar for that matter), you may get indentations in the frame.

Came with a metal wedge to prevent popping off.
Insgesamt bin ich überaus zufrieden, was eher selten vorkommt.
