I need a PHP form (example attached), that once saved, outputs to a 1 PAGE printable file that looks EXACTLY like the form and contents on the page. The formatting must remain completely intact. Ideally, there will be a print button that once clicked, will open a print window with 2 selected as number of copies. The project must be completed by 8/15. If you can not complete the project by 8/15, please do not bid. If you are bidding on this project, please tell me how your strategy to achieve this.
