Wordpress jQuery Plugin

We need a Jquery or html based symptom checker application similar to the one found at: [login to view URL] Our application needs to be a Wordpress plugin that can be deployed in the standard Wordpress fashion. The outline should be a gender-neutral silhouette that can be clicked to provide a second and third set of links for the user to click on. After the second click, we need a default page that offers a “You might suffer from Fibro Myalgia” statement. At the bottom of this page, there will be a button advising the user to “click here for possible solutions.” Once this button has been clicked we will provide a template page that allows us to make a product pitch and comparison between our product and a competing medicine. This will allow us to pitch our product in comparison to the leading competitor. This application needs to be scalable as we plan to add more conditional statements and products over time.
